What is a bone sequestrum caused by trauma?
A bone sequestrum is a medical term used to describe a fragment of bone that, after a traumatic event, has lost its blood supply and died. This fragment separates from the surrounding living bone and cannot heal. Unlike a bone sequestrum caused by an infection, this type is a direct consequence of severe trauma, such as a complex fracture.
During an accident or a high-energy fracture, the impact can damage the blood vessels that supply a portion of the bone. When this interruption of blood flow is complete, the bone tissue dies and becomes a fragment of "dead" bone that the body cannot reabsorb or heal. This fragment becomes an obstacle to normal bone consolidation.
Symptoms and diagnosis
A post-traumatic bone sequestrum often shows up as a complication during recovery from a fracture. Symptoms include:
- Failure to heal (nonunion): The fracture simply does not unite, despite treatment attempts.
- Persistent pain: Chronic pain in the fracture area that does not improve over time.
- Instability: A feeling that the bone is not solid or cannot bear weight.
- Secondary infection: Although it is not the initial cause, a bone sequestrum can become a focus of infection if it is exposed or if bacteria find a path to it, further complicating healing.
The diagnosis is confirmed with imaging studies such as X-rays, CT scans, or MRIs, which clearly show the separated bone fragment with no signs of healing.
