Plantar Fasciitis vs. Heel Spur: Diagnosis and Physical Therapy

Heel pain is one of the most common musculoskeletal complaints we see in our physical therapy practice. Patients often arrive confused, referring interchangeably to their condition as "plantar fasciitis" or "heel spur". While the two are intrinsically related, it is crucial to understand their differences for an accurate diagnosis and, more importantly, to establish an effective treatment plan. As physical therapists, our expertise in the pathophysiology of these conditions and in rehabilitation techniques is fundamental to your recovery.

Plantar Fasciitis: The Cause of Acute Pain

Plantar fasciitis is an inflammation and degeneration of the plantar fascia, a thick band of connective tissue that runs along the sole of the foot, from the calcaneus (heel bone) to the toes. Its main function is to maintain the longitudinal arch of the foot and absorb impact during walking.

Pathophysiology: The main cause of plantar fasciitis is repetitive overload and excessive tension on the fascia. This can be due to:

Repeated microtrauma leads to chronic irritation at the insertion of the plantar fascia on the calcaneus, or along its course. This results in acute pain, especially when taking the first steps in the morning or after periods of inactivity, and at the end of the day.

Heel Spur: The Radiographic Consequence

A calcaneal spur, or heel spur, is a beak-shaped bony growth or protrusion that forms on the underside of the calcaneus, at the point where the plantar fascia attaches.

Pathophysiology: Contrary to popular belief, a heel spur is rarely the direct cause of pain. Rather, it is a consequence of chronic tension and prolonged inflammation of the plantar fascia. The body, in an attempt to repair the damage and increase the insertion surface, deposits calcium at the traction site. It is a radiographic sign that there has been considerable tension on the plantar fascia over a prolonged period.

The key to the differential diagnosis! Many patients with a heel spur experience no pain, while others with severe plantar fasciitis have no visible spur on X-rays. This underscores that the pain comes from the inflammation and degeneration of the fascia (plantar fasciitis), and not directly from the bony spur.

Differential Diagnosis: The Importance of the Physical Therapy Evaluation

An experienced physical therapist will perform a thorough evaluation to determine whether the pain comes from plantar fasciitis or whether there are other contributing factors. This evaluation includes:

  1. Detailed history: Pain history, activities, footwear, aggravating and relieving factors.
  2. Physical examination:
    • Palpation: Localized pain at the insertion of the plantar fascia on the calcaneus, or along its course.
    • Assessment of foot biomechanics: Longitudinal arch, pronation/supination.
    • Range of motion: Ankle dorsiflexion, toe mobility.
    • Muscle strength: Calf and foot muscles.
    • Muscle length: Shortening in the posterior chain (calves, hamstrings).
    • Gait analysis: Observation of footstrike patterns and load distribution.

X-rays can be useful to confirm the presence of a heel spur and rule out other pathologies, but they are not the cornerstone of the diagnosis of plantar fasciitis, which is fundamentally clinical.

Physical Therapy: The Fundamental Pillar of Treatment

Physical therapy is the conservative treatment of choice for plantar fasciitis and for managing the pain associated with a heel spur. Our approach focuses on addressing the underlying cause and promoting healing, not just relieving symptoms.

Physical therapy goals:

  1. Reduce inflammation and pain:
    • Manual therapies: Deep tissue massage of the plantar fascia and calf.
    • Physical agents: Ultrasound, TENS, cryotherapy to modulate pain and inflammation.
  2. Improve flexibility and mobility:
    • Specific stretches: Plantar fascia, calves, soleus, hamstrings.
    • Joint mobilizations: Joints of the foot and ankle.
  3. Strengthen the muscles:
    • Intrinsic foot strengthening exercises: To improve arch support and stability.
    • Calf and glute strengthening exercises: To improve gait mechanics.
  4. Optimize foot biomechanics and gait:
    • Gait analysis: Retraining of footstrike patterns.
    • Footwear education: Recommendations for proper footwear and orthotic insoles if necessary.
    • Taping/functional bandaging: To support the arch and reduce tension.
  5. Patient education:
    • Activity modification to reduce overload.
    • Self-care techniques and prevention of recurrences.
    • Gradual progression of physical activity.
